I'm getting this next in the mail via Gamefly, but I haven't beaten the first game. Just wondering from those who have played it: is it a direct sequel to it? Will I not understand anything that's going on?

As someone who didn't play Okami but played Okamiden myself, I think you'll be fine. ^_^
It refers back to the first game, but I think it's easy enough to play and understand without having played Okami.

^ Exactly. You can play the sequel without having played the first. It'll make more sense if you did, and it makes references and nods to the previous game, but it's nothing game-breaking.

I played Okamiden without playing Okami and I understood things just fine. There is some "hey this happened in the last game! *wink wink*" moments but nothing that would ever lose you.

I played partially through the first game, never finished, never even got to the halfway point, but I loved Okamiden. I understood it fair enough so I would suppose that gamers who haven't played the original should be able to follow the story line okay.

How long is the game anyway compared to Okami?
